Now that you have made an appointment for the Certificado de Delitos de Naturaleza Sexual, it’s time to fill out the necessary document. This guide will show you how to fill out the delitos form.
As mentioned in the previous blog post, the Certificado por Delitos de Naturaleza Sexual is a piece of paper that serves as proof that according to the Spain’s Ministry of Justice’s records, you have not committed any crimes against minors. This is important because most language assistants work with children, from infantil (from 2-3 years old) to bachillerato (from 16 years old until 17 or 18).
How to Fill Out the Delitos Form
1. Download the form here.
The form should have three pages: Page 1 is for the Ministerio de Justicia; Page 2 is your personal copy, and; Page 3 contains the instructions on how to complete the request. That means there is no need for you to photocopy this form.
2. Fill out the form directly from the page.
I suggest you fill out the form first before printing it since you only have to enter your details once. They don’t really care if you write it by hand or type your answers. I still recommend that you check with your regional group or sources for their specific instructions, as it may vary per region.

After filling out the form, either print it directly from the webpage, or save it as PDF and print it later somewhere. You don’t need to make an extra copy. Bring the ID card (passport/TIE) that you used to make the appointment. You can bring a photocopy, but it my experience, it wasn’t necessary.
Should you use a black or a blue pen to sign the document? It doesn’t really matter. Just don’t use any other color, since this is a government form.
